- Find your health condition and fill out a simple online questionnaire
- Your GP will get back to you with feedback and treatment options
Online GP Medical Record Access
This gives you the opportunity to look at a summary of your medical records whenever you chose to do so. Patients who have long term conditions i.e. diabetes, chronic heart disease may find access to test results help them manage their condition more positively. It allows patients access to information held about them providing you with an opportunity to update the GP next time you see them with new information i.e. if you know you have an allergy to a medicine that is not recorded.
Remember the information that is written is to help medical people look after you and in some cases you may not understand everything you see or the medical words used. If you find anything difficult you can ask the Nurse or GP at your next appointment or alternatively refer to NHS Choices website by using this link www.nhs.uk . There is a lot of information on illnesses, improving health and where to find NHS services within your area.
Please beware that your GP may not think it in your best interest for you to look at your GP records online (due to third party information). If this happens your GP will discuss their reasons with you. It is up to your GP to decide if you should be allowed access to your online records
